This training is specific to Residential Provider and will address the steps they should take when an individual has a change in medical condition or is returning home after hospitalization. Restricted Health Condition Care Plans - what is it, why do I need it, and how to obtain one for your client. Understanding what medical conditions are prohibited in your facility, and what conditions are restricted. What are the roles and responsibilities for Providers, Administrators, Licensed staff, and Direct Support Professionals. How to properly prepare a Restricted Health Condition Care Plan and get it approved. Although this is a provider training, some Regional Center Staff are enrolled at the recommendation of their supervisor due to the nature of their job.
